Certificate in Online Instruction: Transcript Request Information
CMU provides complimentary transcript service. You may request an official transcript from CMU after the grade has been posted.
If you need an official transcript of the course(s) you have taken through
Central Michigan University, you may mail or fax your request to the Office
of the Registrar. Be sure to include the following information:
- Name (including any former names used at CMU)
- Social Security Number and/or Student Identification Number
- Date of birth
- Daytime phone number
- Name and address of transcript recipient
- Signature
- Date of request

Fill out
the form completely and mail or fax to the Office of the Registrar.
