IRB Forms for MSA 685 Students
All MSA students are required to complete the Institutional Review Board
process as part of MSA 685, Integrative Analysis, even if the research
project does not involve human subjects. Students are not allowed to
graduate until the process is completed. For those projects involving human
subjects, the use of human subjects in research imposes both ethical and
legal responsibilities upon the institution, the faculty monitor, and the
student researcher. Students cannot collect data until IRB approval is
granted.
Students enrolled in MSA 685, Integrative Analysis of Administration, may
use the downloadable forms listed below or use the forms available in The
Student Guide to the MSA 685 Project. The Guide may be purchased through MBS
Direct (Missouri Book Store). The Student Guide to the MSA 685 Project
provides useful information about writing the MSA 685 project in terms of
form and content. Students will also find expanded descriptions of various
research typologies. IRB policies, procedures, and guidelines are covered
and students will find the "Tips and FAQs" section useful in completing the
IRB paperwork. In addition, sample "exempt," "expedited," and "full board"
IRB applications are included along with sample supporting documents such as
cover letters, consent forms, permission letters, and questionnaires.
Students with projects falling into the "exempt," "expedited," or "full
board" categories should use the application cover sheet, investigator’s
assurance statement, and the MSA 685 IRB Application. After IRB approval is
granted and data has been collected, students should use the end of data
collection report. Students with projects falling into the "not required"
category should use the application cover sheet only. The request for change
in protocol is to be used when a student changes methodology in a project
previously approved through the IRB process. The MSA 685 monitor will
determine which forms are appropriate.
