Transcript Request Form
In order to have your final grades/credit transferred to your school, a transcript request form must be filled out. The form is available to be downloaded at www.registrars.cmich.edu/studentforms/transcriptreq.pdf.
This form should be completed and enclosed with your paper and evaluation. Once a grade has been submitted to the Registrar’s Office, your transcript request will be submitted for processing. The forwarding of your transcript is a free service for normal 1-3 day processing request.
